INSTRUCTIONS:
1. PRINT, READ, and COMPLETE each form
listed below
2. SIGN and DATE where indicated (if under 18
years old, you must include parent/guardian signature)
3. ATTACH a copy of the FRONT and BACK of
your insurance card
4. RETURN all completed forms to:

ALL FORMS MUST BE
COMPLETE AND RETURNED BY AUGUST 1st.
ATTENTION FALL SEASON ATHLETES:
If you did not complete your physical in the spring of the
previous year, you are also required to have your physician
complete the pre-participation physical examination to
establish clearance for sport activity prior to participation
in any activities.
